Indigenous Diabetes Care Navigator with the Pembroke Regional Hospital
The Pembroke Regional Hospital requires an Indigenous Diabetes Care Navigator. This position will be based in the Pembroke Regional Hospital Education Program to work primarily servicing patients in the Renfrew, Petawawa, Chalk River and Deep River areas. This position is offered on a Permanent Part-time basis. Applicants must be available to work all shifts and weekends.
The Indigenous Diabetes Care Navigator will work with Indigenous patients and caregivers and the broader Indigenous community to formulate goals in partnership with the diabetes outreach team, while building and leading multidisciplinary care teams dedicated to meeting those patient goals. They will identify systems gaps, make cross-sector partnerships and advocate on behalf of the patients to assist in meeting their health and social needs. The goal is to improve the healthcare experience for the Indigenous population who have diabetes, pre-diabetes or other related vascular health issues.

Canadian Nuclear Laboratories (CNL) is the operator and licensee of Chalk River Laboratories (CRL), located in Chalk River, Ontario, and within the Algonquins of Ontario unceded Traditional Territory, adjacent to the Algonquins’ most revered waterway, Kitchissippi (Ottawa River).
In the ongoing relationship building between the AOO and CNL, there is an opportunity for Algonquin Businesses to register as a Potential Supplier for CNL procurement opportunities. Through the registration processes, interested businesses can make their company and capabilities known to CNL. Completed registration forms will then be automatically directed to CNL’s Supplier Relationship and Development team where they will be used to develop bidder’s lists as well as used to explore capabilities through invitations to visit or participate in the trade-type show.
